Department of Health Licensee Directory

The Department of Health (HEALTH) Licensee Directory allows anyone to access public contact information about individuals, businesses or healthcare facilities that are licensed by HEALTH. This information is updated on a weekly basis and can be formatted to an Excel spreadsheet or to standard mailing labels.

Instructions for Use

1. Select the licensee(s) you would like to include on your list. (To select multiple lists, use the control key.)
2. Select the license type(s) you would like to include on your list.
3. If applicable, select the secondary license type(s) and/or specialty you would like to include on your list.
4. Click on submit.
5. The licensee count will appear on your screen. (Larger lists may take more time to load.)
6. Select spreadsheet or mailing labels. (If choosing mailing labels, they will be downloaded into a 5160 (Avery) format and a page count will be displayed on your screen.)
7. Download files.
8. After the download screen has opened, you can save the file.
